¿Por qué, si es grave y termina en s, la palabra ítems mantiene la tilde del singular? La regla dice que solo las palabras graves que terminan en consonantes distintas a n o s llevan tilde: ítem (singular), portátil, volátil...

Respuesta: 

Hay una regla adicional que establece que las palabras llanas o graves terminadas en s precedida de otra consonante se tildan: tríceps, ítems, vademécums, afrikáans.
